We are asking you to be included in the Pulmonary Hypertension Association (PHA) Registry (PHAR). This is a national project about people who have pulmonary hypertension (PH). We are asking you to participate because you have PH. This project will collect information from patients with PH who are cared for at PHA Care Centers throughout the U.S.
